Vista Community Clinic has educational programs providing training on self-management of chronic diseases such as diabetes, asthma, and arthritis, with the goal of improving health outcomes for patients with these conditions.
Provide medical visits with Family Nurse Practitioner, visits with nutritionist, health education class series, eye exam, podiatry, labs and some medications.
Provides education on the proper usage of asthma medications and treatments, an assessment of the patient’s home environment, provider referrals and follow up for the families with children ages 0-18 suffering from asthma.
