Public Insurance Application Assistance

If you aren't accessing health care due to finances, you may qualify for public insurance programs. These programs can help you to access free or low-cost health care. You may also qualify for the Cal-Fresh food stamps program which will help you provide nutritious, balanced meals for your family. Certified Application Assistors are available at most clinic sites. You can also make an appointment.

The health insurance programs will allow your family to access:

Who is eligible?

  • Children ages 0-21
  • California Residents
  • U.S. Citizens

Please bring the following to your appointment:

  • Birth certificates for the children
  • Social security cards
  • Proof of income for working parents
  • Two most recent pay stubs
  • Letter from your employer which must include the frequency of payments (i.e. weekly, twice a month, every two weeks, monthly) and payment in gross. "In gross" must be included in the letter.
  • Last year’s tax returns or there month report for self-employed (profit and loss)
  • If you are paying for child care please get a letter from the child care provider stating how much is paid to them monthly.
