Nutrition Support at Vista Community Clinic: When Food Is Part of the Treatment Plan

Good nutrition plays an important role in overall health, but for some patients, it can be especially important as part of managing a medical condition. At VCC, nutrition counseling is available for patients whose healthcare providers determine they may benefit from additional dietary support as part of their care plan.

Nutrition services at VCC are integrated into care for patients with certain health needs, including those participating in the clinic’s “Diabetes Self-Management Education & Support (DSMES)” program, as well as many prenatal patients. In these cases, nutrition guidance can help patients better understand how food choices may affect their health and support their treatment goals.

For example, patients enrolled in VCC’s diabetes management program receive specialized nutrition support as part of their care. Prenatal patients may also receive nutrition counseling to help support a healthy pregnancy and address changing nutritional needs during that time.

In some cases, VCC clinicians may refer other patients for nutrition counseling when they believe it would be beneficial based on a patient’s medical condition or treatment plan.

Nutrition counseling may support patients with:
