Requesting your Records
Did you know you can request your records through your VCC patient portal? It’s fast and FREE!
You can also request your records by filling out an authorization form and sending it to us via email or fax, alternatively, you can drop it off at any of our clinic locations. (Please be advised that all releases are handled by Sharecare Health Data Services; to get a status update, please call their customer service line at 1-800-560-3800)
Please note that Vista Community Clinic requires a completed and signed Authorization for Release of Health Information form before releasing any documents to anyone, including the patient; in certain cases, a patient’s physician, psychologist may also be required to approve the request.
